1. Bangladesh National ID (NID) card 
 2.Copy of birth registration certificate 

If applicable 
1.Copy of Government order 
2.Copy of NOC
3.Copy of previous passport 
4.Copy of technical certificate 
 5.Copy of TIN certificate
6.Proof of retirement date

After fill up application form on line you  will get a  reference ID and password which you  need in future ..

You need to print two copy  online filled  up application form .This filled up application form will  get after sign in with  reference no and password in this
link    http://www.passport.gov.bd 

Affix  your photograph and bank payment slip  on application form..Your photo and other require documents need to attested by the any of following person ...MP,Mayor,Deputy mayor and councellors Gazeted officer, any teacher of public university ,Chairman and vice chairman of upazilla parishad Chairman of 
 union parishad mayor of pauroshova seventh grade and above officer of public and autonomus organization of Bangladesh government ...
After submission all documents properly passport office will take your photograph sameday and will  give  you passport collection date after 10 or 15 days latter.
